recommendation
BACKGROUND: This item was approved at the Administrative Committee on November 19, 2024. Olmsted County has been actively involved in the Any Path Home (APH) initiative since its launch in spring 2024. APH is a collaboration of 10 local agencies dedicated to supporting individuals experiencing homelessness in Olmsted County. Using a collective impact model, APH aims to redesign the community's homeless response system to address existing gaps and challenges. Its mission is clear: to ensure everyone in Olmsted County has a home. A key pillar of the APH initiative is the provision of comprehensive services and support for people experiencing homelessness. To advance this goal, APH partners are working to expand case management capacity. These new case management positions will focus not only on securing housing but also on connecting individuals to a broad range of resources to support their overall well-being. To support this effort, Olmsted County applied for and was awarded a Projects for Assistance in Transition from Homelessness (PATH) grant through the Minnesota Department of Human Services (DHS). This grant, developed in partnership with Doc's Recovery House, will fund two full-time equivalent (FTE) positions, including a Certified Peer Recovery Specialist, to provide outreach and case management services. Funding will also provide direct assistance dollars for expenses such as security deposits, household supplies, or utility connect fees that are required for households to secure and maintain housing. Olmsted County received $432,819 through the PATH grant to cover staffing and direct assistance costs. The grant period runs through June 2026, with the potential for renewal for up to five years.
